If you remove the VTG adjustment ring from the housing (using 3 Allen screws), you will see a flat metal surface (a disc) inside the housing. You will usually see marks on this surface where the VTG wings were located.
If you remove this disc as well, there may also be rust underneath, which could then push the disc upwards.

1. No, I hadn't, everything went great.
2. Unfortunately, the boot on the turbo rotated when opening it, causing it to get caught on the oil line. -> Remove the oil line and replace it.
When dealing with the 3 Torx screws on the VTG linkage, you have to be extremely careful. I had to re-tap the threads afterwards, luckily none of them broke off. I've had that happen before

3. Gasoline, rags, kitchen brush. I removed the metal surface (plate) and first treated it with 180, 400, and 1000 grit sandpaper to remove the scratches as much as possible.
4. Well, I'd say... normal wear marks + rust underneath the metal disc.
