Have you checked it? If there's a leaf stuck in there, it can cause serious imbalances, which I've experienced multiple times with Golf IV and Fabia models. Remove the leaf, and the fan will run smoothly.

Not applicable to the Octavia/Golf IV: In those models, the cabin filter is located at the top of the blower housing, underneath the windshield wiper on the right side. Then comes the recirculation flap, and finally the blower motor.
